To my patients:
All good things must come to an end, and so, I am planning to retire at the end of this year, with 12/16/2022 being my last day for seeing our patients. I have been so blessed to have had so many wonderful and loyal patients for so many years. It has been a joy, an honor, and a privilege to provide this care and watch “my children” grow up, and I have been doubly blessed to now provide care for quite a few second-generation patients.
Patients may select their pediatrician of choice after I retire. I also hope that my website will be a useful resource while transitioning to new medical care.
Patients’ families can obtain their medical records including vaccine records by logging into the Patient Portal and then going to the Medical Records tab. Immunization Records are under the Medical Records Tab, and you may need to scroll to the bottom of the page to get your child’s immunization record. Our office has tried to provide all our patients with their username (which is their account #), and their password (which is initially the child’s date of birth in 8 digits, eg. MM/DD/YYYY, and has frequently been re-set by our office to MMDDYYYY without slashes, so you may need to try both. If you don’t remember your child’s username or even if you haven’t received it, you should be able to use the links for forgot username and/or forgot password in order to retrieve it. And you may try using your child’s date of birth as password or you may reset it. We strongly encourage our patients to go paperless and get their child’s medical records online and do this in a timely manner. Dr. Eisner will keep the portal open until 1/1/2024.
Our office will provide a paper copy or fax a copy of your child’s medical records for a nominal fee to cover our costs. Please reply to this email if you would like to request this, and we will contact you to make arrangements.
Our office needs to stress that our last day for seeing patients will be December 16, 2022. It is important for our patients to make arrangements for their next PCP, particularly those patients that need monthly refills, forms to be filled out, referrals, and routine check-ups and vaccines. All previously scheduled appointments after December 16, 2022, will be canceled.
